This section contains a checklist for completing your application, as well as all tuition information. This document will aid you greatly in submitting a successful application.

PASTOR'S REFERENCE FORM (.pdf/77K)
This form may also be completed by your Bible study leader or your youth pastor. Ask them to complete the form and mail it directly to YWAM Las Vegas. Provide for them a stamped and addressed envelope. Please list their names, addresses and phone #'s on your Application Form.

STANDARD REFERENCE FORM (.pdf/79K)
You will need 2 of these. One of these forms is to be completed by an employer or teacher, and one by a friend. Choose people who know you well and who can be objective. Ask them to complete the form and mail it directly to YWAM Las Vegas. Provide for them a stamped and addressed envelope. Please list their names, addresses and phone #'s on your Application Form. We cannot evaluate your application without all of your references so check on them periodically if you know we have not received
their references.

CONSENT FORM (.pdf/45K)
Please sign each section: the release of liability, consent for treatment, and burial statement. If you are a minor (under 18 years of age) please have your parent or guardian sign as well.

HEALTH FORM (.pdf/53K)
Start on these as soon as possible in case there is anything missing that your family or doctor would need to check into. This can take time and will be out of your hands to hurry along. Make certain everything is completely filled in on these forms. We need this information to be sure you are safe in the climate we live in, to determine what vaccines you may need for outreach locations and choosing appropriate locations should you have any medical needs.
