A complete application packet includes the application with photo, specific school questions, pastor's reference, YWAM leader's reference (general reference), consent form, health form, and a $35 application fee.

We cannot evaluate your application without both of your references so check on them periodically if you know we have not received their references.

Please sign each section: the release of liability, consent for treatment, and burial statement. If you are a minor (under 18 years of age) please have your parent or guardian sign as well.

Start on these as soon as possible in case there is anything missing that your family or doctor would need to check into. This can take time and will be out of your hands to hurry along. Make certain everything is completely filled in on these forms. We need this information to be sure you are safe in the climate we live in, to determine what vaccines you may need for outreach locations and choosing appropriate locations should you have any medical needs.
